Important 2020 Form 1095-B Information
Sutter Health Plus subscribers can get their 2020 Form 1095-B, Health Coverage, online through their Sutter Health Plus Member Portal account beginning February 15, 2021. Subscribers can also request we mail your form to you.

OpenI have not received my 2020 Form 1095-B yet. When are you sending it?
Sutter Health Plus is not mailing the 2020 form to subscribers unless requested. We make the form available through the subscriber’s Sutter Health Plus Member Portal account, and will mail it on request.
OpenHow do I get my 2020 Form 1095-B?
Subscribers can access their form through their Sutter Health Plus member portal account at shplus.org/memberportal. Registration is required. Forms will be available online by February 15, 2021.

Be sure to include your first and last name and your subscriber identification number with your request.
Beginning February 15, 2021, subscribers can also call Sutter Health Plus Member Services at (855) 315-5800 to request a copy sent by secure email or by mail. Member Services is available 8:00 am to 7:00 pm, Monday through Friday.
OpenCan I still get my 2020 Form 1095-B through the Sutter Health Plus Member Portal if my coverage is now terminated?
Yes. The Sutter Health Plus Member Portal allows you to log on up to 18-months after your coverage ends. If you have questions or need assistance with the Member Portal, you can call Sutter Health Plus Member Services at (855) 315-5800. Member Services is available 8:00 am to 7:00 pm, Monday through Friday.
OpenIf I request a mailed copy of my 2020 Form 1095-B, how soon will I get it?
Sutter Health Plus will mail a copy within five business days from the date we receive your request.
