Important 2022 Form 1095-B Information
Sutter Health Plus subscribers can get their 2022 Form 1095-B, Health Coverage, online through their Sutter Health Plus Member Portal account beginning January 30, 2023. Subscribers can also request that we mail the form to them directly.
Frequently Asked Questions
Sutter Health Plus is not mailing the 2022 form to subscribers unless requested. We make the form available through the subscriber’s Sutter Health Plus Member Portal account and will mail it on request.
Subscribers can access their form through their Sutter Health Plus Member Portal account at shplus.org/memberportal. Registration is required.

Yes. The Sutter Health Plus Member Portal allows subscribers to log on up to 18-months after coverage ends. Sutter Health Plus will mail a copy within five business days from the date we receive your request.
