In previous years, Sutter Health Plus mailed Form 1095-B, Health Coverage, to all subscribers as proof of health care coverage. We are no longer mailing the form unless you request it. You can get your 2019 Form 1095-B online through your Sutter Health Plus member portal account. You can also request we mail your form to you.
We changed our process because for 2019 the Tax Cuts and Jobs Act removed the requirement that individuals have qualifying health coverage. For that reason, the Internal Revenue Service (IRS) changed its rules for how health plans provide the form.
Sutter Health Plus is not mailing the 2019 form to subscribers unless requested. We make the form available through the subscribers Sutter Health Plus member portal account, and will mail it on request.
Subscribers can access their form through their Sutter Health Plus member portal account at shplus.org/memberportal. Registration is required. Forms will be available online by March 2, 2020.

Be sure to include your first and last name and your subscriber identification number with your request.
You will receive your form in the mail within 30 days of the date we receive your request.
The IRS advises that you keep the form for your records with your other tax documents.
