A Member’s Journey to Health through Innovative Care
As a single father of four children, Jaime Sanchez admits he hasn’t always made his health a priority—or even checked in regularly with a doctor. Jaime, 40, works as a mechanic in Colma, CA, and between his job and his children, he never found the time to talk to a physician about his weight or concern he might have diabetes, something that runs in his family.
That all changed when Jaime’s employer, Serramonte Volkswagon, began offering Sutter Health Plus to employees. Jaime enrolled in Sutter Health Plus and selected Dr. Yumi DiAngi Taylor as his primary care physician (PCP).
Dr. Taylor, a Sutter-affiliated Palo Alto Medical Foundation PCP, is the founder of TeraPractice, an innovative virtual-first primary care experience that offers easy, mobile access to personalized care. With TeraPractice, a dedicated care team—a PCP, a nurse and a health coach—offers a tailored and seamless experience through the use of video visits, phone consults, email messages and in-person visits when needed.
“I was really impressed with Dr. Taylor’s expertise, her responsiveness and her compassionate nature,” said Jaime. “I liked how she immediately responded to my personal concerns about diabetes and my weight through the convenience of our virtual appointments.”
The TeraPractice model provides members with customizable care focusing on a balance between diet, sleep, stress and activity, making it a great fit for someone like Jaime who is focused on getting healthier.
Jaime began working with his health coach in January and has already lost 25 pounds following her customized guidance on nutrition and physical activity.
“My health coach keeps me accountable through check-in calls every two weeks and we talk about what I’m doing well and what I can do to improve my health,” said Jaime. “I feel way better. I have changed how I cook for myself and for my kids, and we are eating healthier as a family.”
