    Sutter Health Care Management Program

    Health Coaching

    You have access to the Health Coaching program at no cost. This program combines personal life coaching with personal accountability as a way of engaging you in effectively managing your health condition or achieving wellness goals. When you join the program you will work one-on-one with a coach, trained in personal life coaching, to uncover barriers to self-management, link behavior to personal values, and set goals to make healthier lifestyle choices.

    The Health Coaching program offers help with weight management, tobacco cessation, and stress management. Participation is easy. Coaching appointments are by telephone and the first call lasts no more than 20 minutes. You and your coach decide how you are going to work together to address your needs, concerns, and preferences. You can also use the Health and Wellness site to connect with your coach through secure messaging and manage coaching appointments.

    Complex Case Management

    You are eligible for the Complex Case Management (CCM) program if you have chronic conditions or multiple health problems. The CCM team of registered nurse case managers, social workers, clinical pharmacists, and healthcare coordinators can assist you with coordinating care with your providers and understanding your prescription drugs.

    Transitions of Care

    Transition of care services help you safely go from one care setting to another, such as from a hospital to your home or a skilled nursing facility. Discharge planners, case managers, and other healthcare workers begin on the first day of your hospital admission to identify what you will need when you leave and make arrangements. They alert your primary care physician (PCP) of your admission and involve your PCP in your stay and transition. You may opt out of the program when the Sutter Health Care Management team contacts you after discharge.
